Coordinated Intake & Referral Specialist - Full-Time
The Coordinated Intake & Referral (CIR), a.k.a. Connect, Specialist is an independent professional who will be primarily responsible for completing the initial contact interview with pregnant women and parents of infants screened for or referred to Healthy Start or the Coordinated Intake and Referral (CI&R) unit. Through the initial contact, the Intake Coordinator will review identified risk-factors, provide education and resource connections, and determine home-visiting program eligibility. The majority of the work will be performed in-person at a birthing hospital or prenatal provider site located within Osceola County. HIPPA guidelines and client confidentiality must be maintained. Hours can vary and have some flexibility, including working evening and weekend hours in addition to normal business hours. See the full job description.

Fatherhood Program Specialist - Part-Time
The Fatherhood Program Specialist is responsible for the promotion and delivery of the Fatherhood Engagement program. The aim of the Fatherhood Engagement program is to enhance the health and well-being of children and families by supporting, encouraging, and promoting fathers’ roles in their child’s lives. The Fatherhood Program Coordinator is responsible for delivery of evidence-based father-focused parenting curriculum, as one-on-one services, recruiting father participants by engaging in community outreach activities and community education, as well as making and maintaining partnerships with other community service organizations. This position is also responsible for providing direct services to support fathers’ involvement through case management, care coordination, fatherhood initiatives, and life course education.
